To Schedule an Evaluation:

Scheduling an appointment depends on the type of insurance you have or reason for the evaluation.

Please read below to see where you should begin.

1.      For Original Medicare & a Supplemental Plan (Medigap)

  •       A Referral is not needed

  •       Have your PCP or Neurologist fax us your Medical Records, which includes:

    1.      Your Insurance Provider(s) Member ID     

    2.      All Diagnoses

    3.      A Current List of Medications

    4.      Brian Imaging Reports, such as an MRI (preferred) or CT Scans

    5.      Sleep Study Report (if done/available)

  • Once records are received, we will verify eligibility & contact you to schedule an appointment.

    *Please note it may take time for records to be sent, as you may need to sign a consent to release form with the record holder (ie. your PCP/Neurologist’s Office).

2.      If You have Kaiser

  • Have your PCP or Neurologist fax us an Authorization for a Neuropsychological Evaluation.

  • Once the authorization is received, we will verify records & contact you to schedule an appointment.

 

3.      For Commercial or Medicare Part C (Advantage) Plans under HMSA or MDX (Humana/UHC/Aetna)

  •       Have your PCP or Neurologist fax us a referral for a Neuropsychological Evaluation.

  •       The referral should include your medical records (see # 1 above).

  •       Once records are received, we will verify eligibility & contact you to schedule an appointment.

    *Please note it may take time for records to be sent, as you may need to sign a consent to release form with the record holder (ie. your PCP/Neurologist’s Office).

    4. For Letters of Capacity

  • Insurance does not cover this type of testing, as it is not considered “medically necessary” by Medicare guidelines.

  • Fees start at $1,500.00

  •   Have your PCP/Neurologist fax us your medical records (see #1 above) prior to the appointment.

  • Once records are received, we will contact you to schedule an appointment.

    *Please note it may take longer for records to be sent in this situation, as you will need to sign a consent to release form & Letters of Capacity are for the purpose of determining ones decision making ability.

    * Should your Capacity be contested and Doctor is called into court to testify regarding your cognitive capacity (ability), please have your attorney’s office contact us as soon as possible.

    Additional fees will be assessed for court appearances or depositions.

    By including your Attorney, they may have time to coordinate with us when scheduling a court date .

    Please note that we are typically booked out 6-8 weeks in advance. (A fee schedule will be provided prior to testing).

    5. For Traumatic Brain Injury /Court Related Cases

  • Have your Attorney contact our office to request a free consult.

  • Your Attorney should provide medical records/court documents for Doctors review prior to testing.

  • A Fee Schedule will be provided to your attorney prior to scheduling.

    6.      For VA

  • Have your PCP or Neurologist fax us a referral for a Neuropsychological Evaluation.

  • The referral should include your medical records (see # 1 above).

  • Once records are received, we will verify eligibility & contact you to schedule an appointment.

    *Please note it may take time for records to be sent, as you may need to sign a consent to release form with the record holder (ie. your PCP/Neurologist’s Office) .
